Well it depends on your layout. Usually if you're taking a layout someone else made then you're manually pasting code in to the custom CSS box, and the maker will tell you where to put the code for a header. [livejournal.com profile] resplandor made my layout - if you check her journal and find a layout you like, then there are instructions on how to install it and how to add a banner.
